On April 24, 2015, Home to Stay provided a “Lunch and Learn” for Kennedy Hospital Stratford Division’s Acute Care for Elders Unit (ACE). The AlzBetter program was presented to Dr. Terrie Ginsberg (Medical Director of ACE), Melissa Mordecai, APN (Nurse Practitioner) and the entire ACE unit interdisciplinary team. The benefits of our program and the Quality of Life Assessment tool in particular were detailed. We all learned that the ACE Unit and Home to Stay share many goals for seniors; most importantly a safe return to home and the prevention of hospital readmissions. With the ACE Unit’s interdisciplinary approach and AlzBetter’s cutting edge technology, these goals will become a reality for the seniors who avail themselves of Kennedy’s ACE unit and our unique program.
