Introducing the Meaningful Day Program

Group homes and day programs are great, but for some people, staying in their own home is preferable.

At Home to Stay, we believe that a different approach to supporting people living with developmental disabilities is needed. That is why we came up with the Meaningful Day Program, an individualized day programs right in the person’s home and community.

OUR PHILOSOPHY

Individuals want to feel purpose in their lives, and this can best be achieved through the structure of a self-directed day filled with activities that match the their abilities and interests. With the right structure and training in place, individuals can live safely and happily in their own homes, which is where most people prefer to live.

Home to Stay Activity PyramidHOW IT WORKS

A Home to Stay case manager with specialized training assesses each case and comes up with a plan based on the assessment results. By incorporating the Home to Stay Activity Pyramid along with skills such as motivational interviewing, a greater quality of life is offered to participants, and families are finding greater success with far less confrontation.

THE ACTIVITY PYRAMID

The Activity Pyramid is the foundation of a “Meaningful Day.” This tool adapts to the unique needs of each individual, enabling case managers to customize a day that balances these core areas: recuperative, enjoyment, wellness, productive and essential (including personal care, eating/drinking, dressing, sleeping, toileting, etc.).

The Home to Stay Meaningful Day Program is a great way for individuals living with developmental disabilities to have a schedule designed around their specific interests and enables them to engage in the community in a capacity that works for them. Day programs serve an important role in the continuum of care and programming; however, day programs by their very nature are isolating and segregating individuals living with disabilities from the rest of the community.   • A person-centered day program at home
  • 24/7 support with live, responsive staff
  • Spanish-speaking staff
  • Same-day pay
  • Nurse on staff for training and support – medical acuity
  • Certified Home Health Aides
  • Company owner (former DSP) and other staff comes to the home to meet with families weekly or regularly
  • Family training at home
  • We are happy to speak with your support coordination team in person or via Zoom
