Caring for someone with dementia is hard. You may not have to do it alone.

Through Medicare’s GUIDE Program, eligible families can receive dementia care coordination, caregiver support, and respite care at no out-of-pocket cost.

Home to Stay partners with PocketRN and Virtua to help families access:

  • Nurse-led dementia care guidance

  • Caregiver education and one-on-one support

  • Help coordinating care, medications, and community resources

  • Up to approximately 70+ hours of in-home respite care for qualifying families at no out of pocket cost

  • Local Home to Stay caregivers for approved in-home respite visits

Eligibility

Your loved one may qualify for GUIDE if they:

  • Have a diagnosis of dementia, or need help obtaining one

  • Have Original Medicare Parts A & B as primary coverage

  • Are not enrolled in Medicare Advantage, including Special Needs Plans, or PACE

  • Are not currently receiving Medicare hospice benefits

  • Do not live in an assisted living, long-term nursing home or memory care unit

  • Have a family member or unpaid caregiver who needs support

Short-term rehab stays may not automatically disqualify someone. We can help review the situation.

What GUIDE Can Help With

GUIDE is designed to support both the person living with dementia and the family caregiver.

Services may include:

  • In-home respite care at no cost for eligible families

  • A dementia-trained nurse care team

  • Phone or video check-ins

  • 24/7 access to support

  •  Medication review

  • Help coordinating with doctors and community resources

  • Caregiver education and coaching

  • Referrals for additional support

How It Works

  1. Contact Home to Stay. Call us or complete the form so we can learn more about your loved one’s situation.
  2. We help check eligibility. Please have your medicare ID number ready. We’ll help determine whether your loved one may meet GUIDE requirements.
  3. Connect with PocketRN. If eligible, you’ll be connected with a dementia-trained nurse care team for virtual support.
  4. Receive approved respite care. For families who qualify for the respite benefit, Home to Stay can provide in-home caregiver support.

FAQs

Is this really no cost?

For eligible GUIDE participants, covered GUIDE services are provided with no patient cost-sharing. That means no copays for covered GUIDE services, including eligible respite care.

Does Medicare Advantage qualify?

No. GUIDE is for people enrolled in Original Medicare Parts A and B. Medicare Advantage plans, including Special Needs Plans, are not eligible under the current CMS eligibility rules.

Does my loved one need a dementia diagnosis?

Yes. Dementia must be confirmed by an appropriate clinician, but families who are unsure where to start can contact us for guidance.

Is the respite care guaranteed?

No. Respite care depends on eligibility, dementia stage, caregiver need, and Medicare’s annual respite limits.

Who provides the home care?

Home to Stay provides approved in-home respite care through trained caregivers, in partnership with PocketRN’s dementia care team.
